How to Apply for Aayushman Card (Ayushman Bharat Pradhan Mantri Jan Arogya Yojana - PMJAY Card)

The Aayushman Card is a critical component of the Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (PMJAY), a flagship health insurance scheme launched by the Government of India. This scheme aims to provide comprehensive healthcare coverage to economically vulnerable families, ensuring that quality medical services are accessible without financial hardship. The Aayushman Card serves as proof of eligibility for beneficiaries to avail of the scheme's benefits.


Here’s a step-by-step guide on how to apply for the Aayushman Card:

  • Step 1: Check Eligibility

    Before applying, ensure that you are eligible for the PMJAY scheme. Eligibility is determined based on the Socio-Economic Caste Census (SECC) data.

    General Eligibility Criteria:

    Socio-Economic Caste Census (SECC) 2011 Database: The primary basis for eligibility is the SECC 2011 data, which identifies deprived families based on various socio-economic parameters.

    Family Income: Families with an annual income below the specified threshold as per state-specific guidelines.

    Rural Area Criteria:

    In rural areas, the following categories are considered eligible:

    • Households with No Adult Members (16-59 years): Families with no member aged between 16 and 59 years.

    • Female-headed Households: Households with no male adult member aged 16-59 years.

    • Households with Disabled Members: Households where no able-bodied adult is present.

    • Scheduled Caste/Scheduled Tribe Households: Households belonging to Scheduled Castes (SC) and Scheduled Tribes (ST).

    • Landless Households: Households deriving a major part of their income from manual casual labor.

    • Deprivation Criteria:

      • Households living in one-room houses with kutcha walls and roof.
      • Households with no adult members between the ages of 16 and 59.
      • Households without any able-bodied adult member.
      • Households belonging to Scheduled Caste or Scheduled Tribe communities.
      • Households with no literate adult above 25 years.
      • Landless households deriving a major part of their income from manual casual labor.

    • Urban Area Criteria: In urban areas, eligibility is based on the occupation of the family, with 11 predefined occupational categories considered eligible:

      • Ragpicker
      • Beggar
      • Domestic Worker
      • Street Vendor/Cobbler/Hawker/Other Service Provider Working on Streets
      • Construction Worker/Plumber/Mason/Labor/Painter/Welder/Security Guard/Coolie and Other Head Load Worker
      • Sweeper/Sanitation Worker/Mali
      • Home-based Worker/Artisan/Handicrafts Worker/Tailor
      • Transport Worker/Driver/Conductor/Helper to Drivers and Conductors/Cart Puller/Rickshaw Puller
      • Shop Worker/Assistant/Peon in Small Establishment/Helper/Delivery Assistant/Attendant/Waiter
      • Electrician/Mechanic/Assembler/Repair Worker
      • Washerman/Chowkidar
    • Specific Exclusions: Certain households are specifically excluded from the PMJAY scheme:

      • Households with Motorized Two/Three/Four-Wheelers/Fishing Boats.
      • Households with Mechanized Agricultural Equipment.
      • Households with Kisan Credit Cards with a Credit Limit of ₹50,000 and above.
      • Households with any Member as a Government Employee.
      • Households with Non-Agricultural Enterprises Registered with the Government.
      • Households with any Member in Government Employment.
      • Households with any Member Earning more than ₹10,000 per Month.
      • Households Paying Income Tax.
      • Households Paying Professional Tax.
      • Households with Three or More Rooms with Pucca Walls and Roof.
      • Households Owning a Refrigerator.
      • Households Owning Landline Phones.
      • Households Owning more than 2.5 Acres of Irrigated Land with 1 Irrigation Equipment.
      • Households Owning 5 Acres or more of Irrigated Land for Two or more Cropping Seasons.

    • How to Check Eligibility:

      Online Portal: Visit the official PMJAY website and use the “Am I Eligible” feature.

      Call Helpline: Call the PMJAY helpline number 14555 or 1800-111-565 for assistance.

      Visit CSC Centers: You can also visit Common Service Centers (CSCs) for assistance in checking eligibility.


    • Step 2: Gather Necessary Documents

      Ensure you have the following documents ready:

      • Aadhar Card
      • Ration Card
      • Income Certificate
      • Contact Information (phone number)
      • Family Identification Documents (if required)

    • Step 3: Visit the Nearest Common Service Centre (CSC)

      To apply for the Aayushman Card, visit the nearest Common Service Centre (CSC). You can find the nearest CSC by visiting the CSC Locator.


    • Step 4: Application Process at CSC

      Inform the CSC operator about your intention to apply for the Aayushman Card.

      Provide the necessary documents for verification.

      The CSC operator will fill out the application form on your behalf and submit it through the PMJAY online portal.

      After successful submission, you will receive an acknowledgment receipt with your application ID.


    • Step 5: Verification Process

      Your application will undergo a verification process by the authorities. This might include checking your eligibility status and validating the documents provided.


    • Step 6: Receive the Aayushman Card

      Once your application is verified and approved, you will be issued an Aayushman Card. You can collect it from the CSC where you applied or download it from the official PMJAY website.
